Immunization requirements

Per 麻豆视频APK's immunization policy (view on Policies page), 麻豆视频APK students must satisfy vaccination requirements to attend classes on campus and live on campus. 

Students must upload an official immunization record that includes their legal name, date of birth, and dates of administered immunizations. Failure to comply will result in a registration hold on their account.
 

Required immunizations

Immunizations currently required are:

  • Tetanus booster (Tdap preferred or Td) within the last 10 years*  
  • 2 MMRs (measles, mumps, rubella): 2 doses after age 12 months or proof of immunity*

*Minnesota law requires all students born after December 31, 1956, who enroll in a Minnesota college or university to be immunized against diphtheria, tetanus, measles, mumps, and rubella. This law allows for some exemptions. Students who have had measles, mumps or rubella may submit blood titer results to prove immunity. There is no waiver that is acceptable as proof of immunity to diphtheria or tetanus.

Recommended immunizations

麻豆视频APK and the American College Health Association also strongly recommend the following immunizations:

  • Most recent COVID-19 booster recommended by CDC  
  • Annual influenza vaccine  
  • Meningitis vaccine (MenACWY and MenB)  
  • HPV vaccine series  

When are immunizations due? What if I do not submit immunizations?

  • Immunizations should be submitted by August 1 for students enrolling in fall semester and February 1 for students enrolling in spring semester.
  • All 麻豆视频APK undergraduate students must submit their immunizations and/or the appropriate exemption documents before moving onto campus and/or starting classes. If you do not complete the requirement before the deadline, a registration hold will be placed on your student account for the following semester. This means you will not be able to register for classes the following semester.

How do I submit vaccination documentation?

Follow these steps to submit your immunization documentation:

  1. Go to the (If this link doesn鈥檛 work, go to the 麻豆视频APK Counseling and Health Services web page and select the red box 鈥淟og Into Counseling and Health Services Portal.鈥)
  2. At the home screen, select "VIEW MY IMMUNIZATION REQUIREMENTS" (red box).
  3. Complete all the steps under ITEMS REQUIRED FOR CLEARANCE.

                --Immunization record (upload your immunization record here)
                --Individual vaccines listed (enter immunization dates here)
  4. Check the portal after 5 business days to verify your immunization requirement has been met or to check for a message regarding missing immunizations.

How do I locate my immunization records?

  • Please check with your high school or your health care providers office for a copy of your immunization records.
  • Minnesota residents: You can also use the to find your immunization record if your immunizations were done in Minnesota. We will not accept a screenshot of the app. You will need to download your record.
  • Wisconsin residents: You can look up your immunizations in the . We will not be able to accept a screenshot of the app. You will need to download your record.
  • Other states: Check online to see if your state has an immunization database.

Exemptions

  • To request an exemption from the vaccine requirement, students must complete the . This form must be printed and completed for either a medical exemption or a conscientious objection exemption. Upload the completed form to the .
  • Medical exemption: The exemption form must be completed and signed by a physician (MD or DO), physician assistant (PA), or nurse practitioner (NP). Your immunization requirement will not be fulfilled until this signed form is approved by Counseling and Health Services.  
  • Conscientious objection: The exemption form must be completed, signed and stamped by a Notary. Your immunization requirement will not be fulfilled until this signed and notarized form is approved by Counseling and Health Services.

Where can I get vaccinated?

  • Immunizations are available nearby at Lloyd's Pharmacy, other pharmacies, or most primary care medical clinics, all of which should be able to accept insurance.
  • Flu shots are available for students in Counseling and Health Services. Counseling and Health Services cannot bill insurance for immunizations and the cost will be billed to student accounts.
